Rocky Lacrosse

To complete your registration, you will need:    ​

• Insurance , Physician, and Emergency contact information 
• A valid/current USA Lacrosse membership for high school through 6/1/25 (can renew via this registration)
• 9th/11th graders & NEW Rocky players  - COMPLETED Sports Physical Form (2 PAGES!) and online ImPact Concussion Test Completion Confirmation (with player's name and test date on it); YOU WILL NOT BE ALLOWED TO REGISTER without THESE.  The sports physical and concussion test are only valid for 2 years and must be completed again, if they expire before 6/1/25, before you can register.

PLEASE ENTER ALL EMAILS/CELL NUMBERS (Player/Parents/Guardians) IN REGISTRATION THAT YOU WANT TO RECEIVE COMMUNICATION FROM ROCKY LACROSSE!  We cannot add emails to the communication list after registration has ended.

Please note that there are several waivers in this registration.  If your player is 18, he may register himself.  If under 18, a parent or guardian must read and sign the waivers.  

Returning players - Log in using the SportsEngine account you have registered your player with previously.

New players - If you have an existing SportsEngine account you use for other teams, use that one to keep all your registrations for your teams in one spot.

Brand new to SportsEngine?   Create the account in the parent's name/email, and then you will be prompted to add your player's information to that account so that you are connected.
